Dual-Head configuration

AHC hacked up the dual-head display. Read this as a starting point:

The changes are:

• Add a "Virtual" section to /etc/X11/xorg.conf which is the size of the two screens; I believe this initialises the graphics system with enough framebuffer to handle the spanned screen
• Add in /etc/X11/Xsession.d/45custom-xrandr-settings which runs xrandr to configure up spanning - "xrandr --output VGA --left-of TMDS-1"
